How to fill in the forms
- Make sure you have no other applications open.
- Open the form in Acrobat Reader 6 or above.
- Place the cursor in the first box you are required to enter details. The cursor should turn to an I - beam pointer indicating that you can type in the box.
- To move to the next box click on it or press the Tab key.
- After completing the form ensure that all details are correct.
- Print the form.
- Use a blue or black pen to write your signature on the form.
