Proof of age card
The proof of age card is a personal identity card which is available to anyone 18 years or over.
It fits easily into your purse or wallet and shows only those details considered necessary for identification - your name, date of birth, signature and photograph.

To obtain a proof of age card you can collect an application form from any Licensing Centre or Licensing Agent.
Alternatively, you can download it from our Forms section or call Licensing on 13 11 56 or fax us on (08) 9427 8191 and a package will be forwarded to you. Don’t forget to include your full name and address in your fax.
When presenting your completed application form please ensure you have:
- reached the age of 18 years
- one form of primary proof of identification (originals) and one form of secondary proof of identification as shown on the reverse of the application form. If you are presenting copies of your identification they must be certified by one of the people listed on the reverse of the application form.
Note: Student ID cards are not acceptable forms of identification.
If your primary or secondary forms of identification do not contain your photograph, you will need to supply two colour passport size photographs which are not more than 6 months old. Please ensure both your photographs have been endorsed on the reverse side by the person declaring your identity on the application form including the words:
"This is a true photograph of <<the full name of the applicant>>".

Fees
You will also be required to pay

for the issue of the card.
Completed application forms must be presented in person. If you need your proof of age card for a special occasion please remember to allow sufficient time for manufacture and postage of the card after your application is lodged. The process normally takes five to eight working days.
Replacement cards
Should you require a replacement POA card, you will need to re-establish your identity prior to the issue of a replacement card.
You may be required to again present passport sized photographs (not more than 6 months old) if your primary or secondary ID does not provide a photo.
